Wouldn't this require a similar system to run flats where the sensors are in the wheels or something?

You would need a sensor most new cars come with tpms I think it became mandatory on cars after a certain year. Mostly they are connected to the bottom of the valve (inside the tyre) usually quite a big thing but are actually getting smaller. The aftermarket tpms kits use a valve cap for the sensor.
